Most fish oil on the market is rancid, under-dosed, or in the wrong chemical form. Here's how to choose an omega-3 that actually moves your blood markers — and the five products worth buying.
In 2021, a study quietly published in the *Journal of Nutritional Science* tested 47 fish oil products purchased from major US retailers. The researchers measured oxidation markers — peroxide values, anisidine values, the standard tests for rancidity. The results were grim. Eighty-three percent of the tested products exceeded the international rancidity threshold for fish oil. Most of them by a wide margin. The fish oil sitting in your kitchen cabinet right now, if you bought it from the cheapest aisle of your grocery store, is statistically more likely to be rancid than fresh.
This isn't an indictment of omega-3 supplementation as a longevity intervention — the evidence for omega-3s is overwhelming. A 2021 meta-analysis of 17 prospective cohort studies covering 42,000 participants found that people in the highest quintile of omega-3 blood levels had a 16% lower risk of all-cause mortality than those in the lowest quintile (Harris et al., *Nature Communications*, 2021). The effect size is roughly comparable to quitting smoking or getting your blood pressure under control. Omega-3 supplementation doesn't replace a Mediterranean diet rich in fatty fish, but for the 95% of Americans who don't eat fatty fish twice a week, supplementation is one of the best-studied longevity interventions you can buy at a drugstore.
The catch is that most fish oil is junk. To get the documented longevity benefits, you need a product that meets four criteria simultaneously: (1) high enough EPA + DHA per serving to actually reach therapeutic blood levels, (2) the triglyceride or re-esterified triglyceride form for absorption, (3) third-party purity testing for heavy metals, PCBs, and rancidity, and (4) the right dose for your goal. Most products fail at least two of those tests.
After three years of label reading, third-party assay reviews, and cross-referencing with the omega-3 index data from my own bloodwork, here are the five products I trust enough to put in my own daily protocol.
Before the picks, the four things that distinguish fish oil that works from fish oil that wastes your money:
Total EPA + DHA per serving. This is the only number that matters for therapeutic dosing. Many cheap fish oils show "1,000 mg of fish oil" prominently while the actual EPA + DHA content is buried at 180 mg + 120 mg = 300 mg per softgel. To hit the 2 g/day longevity target, you'd need 7 of those softgels. Look for products that deliver at least 1,000 mg combined EPA + DHA per serving.
The chemical form. Fish oil comes in three forms: ethyl ester (EE), triglyceride (TG), and re-esterified triglyceride (rTG). Triglyceride forms absorb roughly 70% better than ethyl ester forms in head-to-head studies (Dyerberg et al., *Prostaglandins, Leukotrienes and Essential Fatty Acids*, 2010). Most cheap fish oil is ethyl ester because it's cheaper to produce. Always check the supplement facts panel — it must specify TG or rTG.
Third-party certification. The two certifications worth caring about are IFOS (International Fish Oil Standards) and USP (US Pharmacopeia). Both verify purity (heavy metals, PCBs, dioxins all below international limits) and freshness (peroxide and anisidine values within acceptable ranges). A current 5-star IFOS report is the closest thing to a guarantee you'll get from this category.
The omega-3 index target. The most useful biomarker for fish oil dosing is the omega-3 index — the percentage of EPA + DHA in red blood cell membranes. A value below 4% is associated with increased cardiovascular risk; above 8% is the longevity target. Most untreated Americans test between 3% and 5%. Reaching 8%+ requires roughly 2 g/day of combined EPA + DHA for most people, sustained for 4 to 6 months. The home test kits cost $50, and they're worth doing once.
For most people, the right answer is a triglyceride-form softgel that delivers 1,000+ mg combined EPA + DHA per serving with a current IFOS 5-star certification. This is what I take. The combination of high dose density (so you only swallow 1 to 2 softgels per day), absorption-optimized form, and third-party purity verification covers all four criteria above.
Two softgels per day with your largest meal of the day (typically dinner) hits the 1.4 g target consistently and removes the variable of measuring liquid doses.
If you're going to be taking 2 to 4 grams per day for the rest of your life — which is the protocol most longevity physicians recommend — the per-gram math starts to matter. Liquid fish oil is dramatically cheaper per gram of EPA + DHA than capsules (typically 40 to 60 percent cheaper) because you're not paying for capsule shells and capsule manufacturing. The other advantage: dose flexibility. You can scale up to 3 or 4 g/day without swallowing 8 horse pills.
The downside is texture and storage. Liquid fish oil must be refrigerated after opening, and even the best lemon-flavored versions take some adjustment. If the bottle ever smells fishy or rancid, throw it out. Buy small bottles (16 oz or less) so you finish them in 4 to 6 weeks.
For travel, for people who don't want to deal with refrigeration, or for anyone who finds liquid fish oil texturally unpleasant, capsules remain the practical default. The version I'd buy is a moderate-dose IFOS-certified softgel that you can scale to your needed dose.
Krill oil is a different beast. It delivers EPA and DHA bound to phospholipids rather than triglycerides, which some studies suggest improves absorption into red blood cell membranes (the metric that matters for the omega-3 index). It also comes naturally with astaxanthin, a potent antioxidant that protects the omega-3 fatty acids from oxidation. The downsides are dose density (krill oil is typically lower-dose per softgel than fish oil) and cost (significantly more per gram of EPA + DHA). Worth considering if you want a more bioavailable form and don't mind paying for it.
Algae oil is the original source of EPA and DHA in the food chain — fish accumulate these fatty acids by eating algae. Modern algae oil supplements deliver EPA and DHA directly from cultivated algae, with no fish involved. The data shows algae omega-3 raises blood EPA and DHA levels comparably to fish oil at equivalent doses. For vegans, vegetarians, anyone who can't tolerate fish products, or anyone concerned about ocean sustainability, algae oil is the clear answer.

Take it with your largest fat-containing meal. Omega-3s are fat-soluble. Taking them on an empty stomach or with a low-fat meal cuts absorption by 30 to 50 percent. The biggest meal of the day — usually dinner — is ideal.
Aim for 1.5 to 3 grams of combined EPA + DHA per day. This is the dose range that consistently moves the omega-3 index into the >8% target range over 4 to 6 months. Cardiovascular trials use 2 to 4 g/day. People with elevated triglycerides or active inflammation may need higher doses (work with your doctor).
Test the omega-3 index after 4 months. This is the only reliable way to know if your protocol is working. Home test kits from OmegaQuant or similar providers cost about $50 and require a finger-prick blood sample. Adjust dose based on results.
Refrigerate after opening if liquid; cool dark storage if capsules. Heat and light accelerate oxidation. If your fish oil ever smells or tastes fishy or rancid, it's oxidized and should be discarded.
Pair with vitamin E or astaxanthin. Both protect omega-3 fatty acids from oxidation in the body. Many high-quality fish oils already include vitamin E for this reason; if not, a separate dose helps.
How much omega-3 should I take per day?
For longevity, 1.5 to 3 grams of combined EPA + DHA per day is the standard target. For elevated triglycerides or active inflammation, 2 to 4 g/day. The total dose matters more than the number of capsules.
Triglyceride vs ethyl ester — does it really matter?
Yes. Triglyceride and re-esterified triglyceride forms absorb roughly 70% better than ethyl ester forms in head-to-head trials. For the same daily EPA + DHA target, you'd need substantially more ethyl ester product. Always check the supplement facts panel.
Is fish oil bad for the heart? I've seen mixed studies.
The mixed results in early trials (ASCEND, VITAL) used low doses (around 1 g/day). Higher-dose trials (REDUCE-IT at 4 g/day, others) consistently show cardiovascular benefit. Meta-analyses of all available trials (Hu et al., *JAHA*, 2019) show roughly 9% reduction in cardiovascular mortality with omega-3 supplementation. The dose matters.
Do I need to refrigerate fish oil?
Liquid fish oil — yes, after opening. Capsules — generally no, but cool dark storage extends shelf life. The moment your fish oil smells or tastes off, it's rancid and should be discarded.
Can I get enough omega-3 from diet alone?
Possible but difficult. Two servings of fatty fish per week (salmon, sardines, mackerel) provides roughly 250 to 500 mg/day of EPA + DHA — well below the 1.5 to 3 g/day longevity target. Most adults benefit meaningfully from supplementation on top of diet.
Is krill oil really better than fish oil?
The phospholipid form in krill oil may have absorption advantages, particularly for raising the omega-3 index. The clinical difference in trials is modest. If you can afford it and like the idea, krill oil is fine. If you're optimizing for cost-per-gram of EPA + DHA, fish oil wins.
For most longevity-focused buyers, a triglyceride-form fish oil providing 1.4 to 2 g/day of combined EPA + DHA, IFOS-certified for purity, taken with your largest meal of the day, is the right baseline. Liquid is dramatically cheaper per gram if you're going high-dose. Capsules win for travel and convenience. Algae oil is the best vegan option. Krill oil is the premium pick if absorption is your priority.
Whatever you choose, test your omega-3 index after 4 months. The whole point of supplementation is to move a biomarker into the target range, and the only way to know it's working is to measure.
